Take I-44 to Carthage Exit (Highway 71). Take the first Carthage Exit (Garrison) pass Walmart on your Left. Go into the round about (You can only go Right). Take the second exit out of the round about (Grand Street). Take a Right at the Stop sign (Fairview Street). After you pass Fairview Elementary School on your Left there is a road on your Left called Buena Vista, turn there. You'll see the sign for Carthage Health and Rehabilitation Center on your Left.
